Candidates applying for admission to U.P. State Medical Faculty are required to apply online only through the official website: https://upsmfac.org.
Before filling up the Online registration/application, the candidate must be ready with the following documents:
- A scanned image of Recent Colored Photograph of the Candidate’s Name & Date of Photo is mentioned below on the photograph. (File size of max 100KB in JPG/JPEG format only)
- Scanned Signature of candidate. (File size of 80KB (maximum) in JPG/JPEG format only)
- E-mail Id
- Mobile No.
- Colored Scanned Copy of Mark sheets and certificates for 10th and 12th
- Colored scanned copy of Caste Certificates (if applicable)
- Colored scanned copy of Domicile-Certificate (if applicable)
